Event Self-Evaluation
Students, below is the event self-evaluation. This is to be completed after you have finished your main event.  You are to answer questions regarding the event for which you had an “event operations” role.

All students are required to submit an evaluation after the conclusion of the event (see syllabus). Fill out all of the information on this form before clicking, "Submit." Incomplete forms will not receive credit.
This self-evaluation asks students to submit information in the following areas:

More Information:
You have just been part of a larger team trying to organize and run an event effectively. Certainly, you have a number of thoughts related to your individual performance, the group's performance, and the overall implementation of the event. This evaluation is designed to 'pick your brain.' Please detail your thoughts, and when possible, provide specific examples of situations, etc. that illustrate your point.
Note that this form sends me an email (date/time stamped) of all of your answers, and thus, should be submitted on time. Late submissions will be deducted accordingly. Also, note that if you submit this form more than once, you MUST COMPLETE the ENTIRE FORM each time.
As with all forms submitted to me, the student should paste the contents of this COMPLETED form in a Word document to retain and save in their possession. This will ensure that the student has copies of all materials should a form need to be resubmitted.
INDIVIDUAL EVALUATION
Whether or not a role "title" was defined, give your role in the event a "title" (e.g., marketing group coordinator, sponsorship sales representative, event registrar, event grounds crew, waterboy to Professor Wilson, etc.) (required)
Discuss your pre-event responsibilities. Be as detailed as possible so as to "flesh out" your role in bringing this event together. Responsibilities could include anything from phone calls, to making materials, to organizing meetings, to photocopying, to website development, to field measurement in the snow, etc. This section should be at least 5-7 sentences to earn credit. (required)
Discuss your in-event responsibilities. As with number 2 above, be as detailed as possible. Account for everything under your purview at the event. This could include anything from registration, to organizing officials, to recording scores, to communication with team captains, to grilling Professor Wilson a great hot dog, etc. This section should be at least 5-7 sentence to earn credit. What did you contribute to the whole process? That is, what strengths did you bring to the table in organizing and implementing this event? (required)
Discuss the quality of your work. Provide a specific example that emphasizes that quality. (required)
What kind of effort did you put forth for this event? Provide a specific example that emphasizes that effort. (required)

Who was/were the sponsor(s) of the event? Discuss what benefits this/these sponsor(s)received by being involved in the event. This answer should be 5-7 sentences long to earn credit. (required)
In the post-event, to the best of your knowledge, is someone putting together fulfillment packages to give to the sponsor(s)? (The representatives that landed each sponsorship account are normally responsible for this). If someone were putting together a fulfillment package for the sponsor(s), what kind of items do you believe should be included in it? This answer should be 5-7 sentences long to earn credit.
